I am suffering from stomach heat since 2 weeks. Hence I was prescribed sompraz dsr and acinil o. However post taking sompraz dsr I am having weakness and hand shakiness even after having food. This tablet composite of PPI esomeprazol. Does this can cause low bloody sugar in patient without diabetes. As these symptoms improve post having biscuits as per may observation for last 3 days. Please explain. As google show in rare cases it does lower blood sugar. I had my recent blood test. My HB, vitamin d, thyroid, fasting sugar and average glucose levels, all are normal.

In Sompraz DSR.... Domperidone is present. domperidone can cause extrapyramidal symptoms (EPS), but it is a very rare side effect. If it is given in high dose or if any other medicines are used along with domperidone, it can cause tremors / shaking of hands. It reaches its highest concentration in the bloodstream around 30 minutes to 1 hour after consumption, which is when you might have experienced these symptoms..... And immediately body recognise and starts its action against it ( may be then you must have had biscuit and you feel your symptoms getting better.).

you don't need domperidone as you don't have vomiting. just take plain sompraz or shift to pantoprazole 40mg without domperidone. continue acinil o syrup. that will be enough

1. Can Esomeprazole cause hypoglycemia?
Very rarely, yes- but it's not a common side effect.
PPI like esomeprazole are not known to directly lower blood glucose. There are only isolated case reports of hypoglycemia, mostly in patients on multiple drugs or with liver/kidney issues. In a non-diabetic with normal fasting & average glucose, PPI alone causing hypo is unlikely.
2. What else could cause "weakness + shakiness after food"?*

Domperidone is more likely to be related here. Possible causes:
- Reactive/Dumping-type symptoms: If "stomach heat" is due to gastritis/GERD, eating small amounts like biscuits gives quick relief. That suggests gastric irritation, not true hypo.
- Electrolyte / B12 / Mg deficiency: Long term PPI use can affect B12, Mg. But 2 weeks is too short.
- Anxiety/stress component: "Stomach heat" + shakiness often overlaps with anxiety.
- Dehydration or poor intake due to gastritis symptoms
3. Why do biscuits help?
That points more towards gastritis/empty stomach acidity or functional dyspepsia. Food buffers acid → symptom relief. True hypoglycemia would need glucose monitoring
