Posterocentral disc extrusion at L4-L5, L5-S1 level causing indentation of ventral thecal sac. Ligamentum flavum hypertrophy at L4-L5, L5-S1 levels.

Dear Practo user , There is multiple pathology in MRI as you mentioned . There is extrusion , LF hypertrophy causing canal stenosis at multiple levels . The aim is to find right disc causing radiculopathy ( radiating pain ) , despite multiple disc involvement, all disc doesn’t cause pain at same time , hence differential diagnosis is required with clinical assessment of patient . Once diagnosis is established to find pain generator ( source of pain ) , it can be treated by interventions ( from nerve root intervention to endoscopic spine procedure and open spine surgery ). There is very little chance than disc extrusion or lig flavum hypertrophy improving own it’s own despite taking pain killers.

Consult interventional pain specialist or Orthopaedician trained with spine fellowship for finding source of pain and addressing that pain generator appropriately
