U have chronic generalised periodontitis that is involving more than third of ur gums around teeth affected. U will need to get a cleaning done and resolve the gingival recession that is reduction of ur gums to a bare minimum. Ur teeth are shaking that means it’s mobile. Now u will have to go to a periodontist or a general dentist who is well versed in treating pockets. It will usually involve making a incision cleaning root and bone of bacterial remnants. Then placing bone material in ur bone and gums and putting the gum in higher position followed by periodic evaluation. If ignored tooth can reach to grade 3 mobility and will come off into ur hands at any point of time after grade 3. U need a quick prevention strategy and visit an essential service dentist in ur area

Your loose teeth is because of calculus deposits. This is called periodontitis. The fact that there is calculus deposit even after getting it cleaned twice means that there is lack of brushing in the lower front tooth region.
Next Steps
Get your teeth cleaned again. Use an antiplaque toothpaste at night before going to bed, like x-tar.
Health Tips
When you brush your teeth try to clean all surfaces. Stand in front of a mirror, look at your teeth while brushing. This will help see which all surfaces your brush is touching and/ or missing.
