Levera to Briv Switch
For past 5 years, I was on Levera 500 mg twice a day. But due to side effects like low concentration, emotional numbness, low mood, lack of motivation etc, my neurologist has switch medicine to Brivaracetam 50 mg in the day and 100 mg in the night. I have been on Briv since a week now.

If a patient on levetiracetam has:
✅ No breakthrough seizures
✅ Good seizure control for the intended duration
✅ No significant adverse effects, especially behavioral/psychiatric effects
✅ Good adherence
✅ No clinically important drug interactions
✅ Renal function/dose are appropriate
then there is generally no need to switch levetiracetam to brivaracetam simply because brivaracetam has higher SV2A affinity.
Practical approach
LEV working well + tolerated well → Continue LEV.
Consider switching to brivaracetam mainly when there is a reason, such as:
behavioral adverse effects with LEV
inadequate seizure control despite appropriate LEV therapy
tolerability problems
a specific clinical reason to prefer BRV.
Higher SV2A affinity ≠ automatically better clinical efficacy.
So in a stable, seizure-free patient who is tolerating levetiracetam well, changing an effective ASM can itself introduce unnecessary risk of adverse effects or loss of seizure control.

Switching from Keppra (levetiracetam) to Briviact (brivaracetam) often provides rapid physiological transition, but noticing improvements in mood or seizure control can take anywhere from a few days to a couple of weeks.
You are already on right dosage of medication. Continue the same. Wait for reversal of your symptoms. If persistent even after couple of weeks,look for other causes for your symptoms

Brivaracetam is closely related to levetiracetam, and the medicine reaches effective levels quickly, but seizure control and improvement in side effects may take a few weeks. The switch and dose should be guided by your neurologist; do not stop or alter either medicine yourself.
