Aapke husband ka BP ab 96/67 aur 115/82 aa raha hai, matlab dawa zyada pad rahi hai - garmi mein aisa hota hai kyunki pasine se paani aur namak nikal jata hai. Aise BP par teen dawaiyan full dose mein continue karna theek nahi, chakkar aane ya girne ka risk badhta hai. Lekin dose khud se half karna bhi galat hai - kaun si dawa kam karni hai ye ECG, kidney function aur home BP readings dekh kar decide hota hai.
Next Steps
Aaj se subah aur shaam BP likhna shuru karein - 7 din ka record banayein, aur namak-paani jab bhi BP 100 se neeche ho. Agar chakkar aaye, dhundhla dikhe ya wo behosh jaise lagein to turant hospital le jaayein. Ye 7 din ka chart le kar mere saath Practo par consult book karein ya Synergy Multispeciality Hospital, Miraj aa jaayein - main dose safely adjust kar dunga, aap khud dawa half na karein.

BP of 180/110 was very high. It was ideal to start dual antihypertensive medicine.
But when BP is getting into control or when it is falling below normal level, we need to adjust dose accordingly.
If he was using met xl 50, better to make it 25 rather than breaking the tablet.
You didn't mention whether u are continuing telma 40 or not. So it's little difficult to advise.
For personalized management you need to provide BP in the morning as well as in the evening. Also your husband's symptoms are needed.

115/82 normal hai, tablet half karna theek nahin, 15 din 50 ki jagah 25mg sirf metoprolol aur 40mg Telmisartan chalne dijiye, b.p. stable, normal rahta hai to isi dose men chalne dijiye.
Telmisartan 40mg morning and 40mg evening can be tried
Gradually taper and stop the other medicine since itcontains metoprolol which can’t be stopped suddenly
Requires a proper assessment to understand the underlying cause and guide you.
