Ischemic stroke and hyperunicemia
My mom suffered an ischemic stroke and has hyperuricemia. Despite taking her daily prescribed medications, she is experiencing severe weakness, a heavy head with headaches, and intense dizziness that prevents her from standing or walking. She is also suffering from an active gout flare and ongoing leg pain. She curently under nuerology treatment, the swollen leg is reduce, but seem like not helping much it's been 3 month, her toe still has pain sometimes it goes, I don't know whether I should see another specialist for hyperunicemia or Internal Medicine specialist .I want a doctor that can safely adjust her medications to relieve the dizziness and provide a stroke-safe treatment for her gout flare

This is exactly the kind of case an internal medicine physician should be handling, not more specialists in parallel. Her dizziness and heavy head at 73 are usually blood pressure dropping too low on the current tablets or over-sedating drugs, not the stroke itself. And an active gout flare with a swollen painful leg needs treatment that does not clash with her blood thinner - NSAIDs like diclofenac are the wrong choice here.

Take her to an internal medicine physician with all her medicine strips, and carry a week of BP readings sitting and standing, plus uric acid, creatinine, sodium and HbA1c. For the flare, low-dose colchicine or a short steroid course is usually the safer route, and febuxostat is started only once the flare settles.
