Hi Dear!
I felt like you were crying while you were writing it.
I am an oncologist. I have seen so many terminal patients. It is not an easy sight. What you are going through, is not easy.
Palliative care is where we believe what can we do best to ease the breaths of the patient, ease his pain, where he can atleast eat and drink on his own ,go to washroom on his own.
Next Steps
So, The condition which you have mentioned looks like a metastatic disease requires palliative care.

Even though the details you have provided is not specific like the CT and biopsy reports, from the points you have mentioned it can be understood that he has an advanced disease. It would be great if you could visit with him to his treating oncologist next visit, so that you can discuss (if patient allows) about how to care for him best.
If it’s a metastatic disease and he is in a poor general condition, probable palliative care alone would be apt. But finally it all depends on the discussion between the patient and his doctor.
Need some extra information to say the stage and time period he might survive
With the history you've given looks like his performance status is poor which is a bad sign for survival and to take any treatment
Can tell you more clearly only if I can see the history investigations and the current treatment..
Clinically looks like stage 4
Rest will get clear picture as with the reports
The survival will depend upon the reports of pet ct and ongoing treatment regimen
There is too much too explain !
I am a surgical oncologist!
It would be better if you could arrange and online consult and share all documents so that i can give you a complete picture !
