No need to remove foam from top of your buttermilk..Normal sigmoidoscopy report has already ruled out serious underlying colon diseases like ulcer, tumour etc..so dont take too much stress..your long standing IBS and migraine shows that your Vata n Pitta doshas are imbalanced..that's why you must be advised to avoid black pepper, chilli etc.
Next Steps
You must visit Ayurveda Practitioner for proper guidance and your prakriti evaluation..till that time..continue taking buttermilk with roasted jeera n black salt during lunch time..Don't take it at night time

* Suffering from IBS since 15 years. * It became a deerghakaalika vyadhi for you.... * In Ayurveda, IBS (Irritable Bowel Syndrome) is considered under Grahani roga, Agnimandhyam and due to imbalance of vata and pitta doshas. * IBS occurs due to psychological stress, anxiety where more brain - Gut connection mechanism will involve. * Release of more seratonin or alternative release of serotonin alters gut motility either to constipation or Diarrhoea. This is a clear cut example for prakupita vata dosha where vishamagni is purely to be treated. * Apana vayu, Samana Vayu, Vyana vayu and Prana vayu (disturbance of all these vayus) + Associated with Ama?? Needed some extra care or some extra questions to be cleared.
Next Steps
* So, Ayurveda aims for correction of vishamagni and all disturbed mechanisms of vayu.... * Many rasoushadhi's, parpati's, aristas etc., will help you to ease your condition. * But before using any medication.... clear cut data of prakriti of a patient, agni of a patient etc., to be evaluated. * Since u r suffering from 15 years, it takes time for cure and needs some patience.... Thank You

Regarding the buttermilk foam — no need to remove it, it's just air from churning and completely harmless. Continue buttermilk, it's an excellent probiotic for IBS. For your 15-year IBS with known triggers: the most effective dietary approach is a Low-FODMAP diet (avoid onion, garlic, certain fruits, lactose, wheat in addition to your current triggers). Full cream milk likely bothers you due to lactose — try lactose-free milk or curd/buttermilk instead (already fermented, lower lactose). Adding a daily probiotic capsule (VSL#3 or Lactobacillus/Bifidobacterium combination) alongside buttermilk gives added benefit. Enteric-coated peppermint oil capsules (taken before meals) significantly reduce IBS colon spasms. For pain episodes, an antispasmodic like mebeverine or dicyclomine taken as needed helps. Regarding migraine + IBS: both are linked through the gut-brain axis — stress, poor sleep, and skipped meals trigger both. Magnesium glycinate 300mg daily can help reduce frequency of both migraines and gut spasms.
Next Steps
Continue buttermilk daily. Add probiotic capsule. Try enteric-coated peppermint oil before meals. Take antispasmodic for acute IBS pain. Follow Low-FODMAP diet. Start Magnesium glycinate 300mg at night — helps both IBS and migraine. Consult me on Practo for further guidance.

15 years of IBS needs more than just avoiding foods. A normal sigmoidoscopy is reassuring, but persistent pain and multiple food triggers need a proper evaluation of your bowel pattern, digestion, diet and stress/migraine association. You do not need to remove the foam from buttermilk routinely. Ayurveda with individualized diet and medicines can help manage chronic IBS effectively. Consider a detailed consultation rather than adding more food restrictions on your own.

A normal sigmoidoscopy is reassuring and makes serious structural diseases (such as ulcers, tumors, or inflammatory bowel disease in the examined area) less likely. Since you've had IBS symptoms for 15 years, your symptoms are likely due to visceral hypersensitivity and altered gut motility, where certain foods trigger pain despite a normal bowel examination. The fact that black pepper, chili, garam masala, and full-cream milk trigger your symptoms is not unusual. If avoiding these foods helps, it is reasonable to continue avoiding them. Regarding buttermilk: If you tolerate it well, you do not need to remove the foam. The foam itself is not harmful. If the buttermilk is made from low-fat curd and does not worsen your symptoms, it is generally a better option than full-cream milk for people who are sensitive to dairy. Since you also have migraine, it is important to note that some foods (aged cheese, chocolate, caffeine excess, processed foods, MSG, and irregular meals) can trigger both migraine and IBS in some individuals. From an Ayurvedic perspective, long-standing IBS often correlates with disturbances in Agni (digestive fire) and Vata dosha, though the exact assessment depends on whether your symptoms are diarrhea-predominant, constipation-predominant, or mixed. Treatment is individualized and may include: Correction of Agni. Dietary regulation. Lifestyle modifications. Classical Ayurvedic medicines based on your constitution and symptoms.
Next Steps
Since your symptoms have been present for many years, you must consult us, especially if your symptoms change or worsen. Also seek medical review if you develop: Blood in stools. Unintentional weight loss. Persistent fever. Anemia. Night-time diarrhea or pain that wakes you from sleep. For a personalized Ayurvedic treatment plan addressing both IBS and migraine, you may consult us at 9 3 1 5 6 6 1 5 6 5.
Health Tips
Eat small, regular meals rather than large meals. Avoid prolonged fasting. Keep a food and symptom diary to identify personal triggers. Stay well hydrated. Manage stress through yoga, meditation, or breathing exercises, as stress commonly aggravates both IBS and migraine. With the right combination of diet, lifestyle measures, and individualized treatment, many people with long-standing IBS achieve good symptom control and a much better quality of life. Thank you
