Have a TSH level of 12.3. I was diagnosed with TSH level of 7 earlier last year but that was treated completely to TSH level 5. Now TSH level raises again.
I am attaching my recent Practo Report. Also I am Vitamin B and D deficient.

You need to consult personally for detail and better management. Yes, u need treatment for this to prevent some future problems related to these abnormal reports.
Were you taking any medications for thyroid issues
Also what are the levels of vitamin D3 and B12

When we treat hypothyroid we try to achieve a tsh level at least near 2 before we start reducing the dose any ways. I suggest you start taking thyroxine 75 mcqs 30 mins before your bf. Recheck tsh after a month.
For your b12 take T. Benformet forte one tablet daily for one month. The rich source of b12 is non veg. If your veg then continue taking this tablet and consult a nutritionist who can give you a proper diet plan, so that in future you will not have any of this deficiencies.
Cap. D - rise 60k one cap in the afternoon, then the next dose after 15 days one cap. The cheapest source of vit d is sunlight so expose yourself to sunlight for 20 mins daily between 8-10 am.

Regarding your thyroid if things do not work out consult an endocrinologist.

It seems like you have hypothyroidism. But total T3 and T4 values are not conclusive. You need to go for free T3 and T4 levels.
Are you on any medications? What are your present complains? Weight gain? Decreased appetite? Etc.
Take consultation with the reports mentioned. You will need medications otherwise.
Yes ur TSH level is too high. R u taking any medicine? If yes, what's the dosage? Increase the dosage.
