For sugar take high fiber diet, low fat, stop sweets items and sweet fruits completely...
Can take broken wheat, whole wheat rotis, cooked brown rice quarter cup and avoid roots and Tuber vegetables completely, include Low GI complex carbohydrates, moderate plant proteins, low fat, Omega 3 and 6 rich foods, walnuts, almonds, seeds, figs, oats, rye, millets, buckwheat, barley etc..
For bp low sodium, low/ less salt like a pinch, rest and relax much, morning and evening walks, good nigh sleep, enough water intake etc..
Meditation is very helpful in controlling high bp...and sugar too...
