I’ve been on prescribe topical finasteride + minoxidil 5 percent for 4 years. It worked initially, but I’ve started thinning again, mainly crown/temples.
Derms are pushing PRP and want me stop finasteride and minoxidil, but I’m skeptical since it doesn’t block DHT.
Does continued thinning mean DHT suppression isn’t enough anymore?
Would it make more sense to switch to oral finasteride or dutasteride instead of PRP?
What’s the logical next step before considering a transplant?

Oral medications do help.
You can also use dermaroller or stamp alongwith your topical minoxidil. And use a 10%minoxidil with finasteride.
PRP is great option but do be mindful that it might be a little painful for some patients, so get it done under a well trained doctor.
I think Yes for sure oral finasteride or dutasteride will give you better outcome.
PRP is at best of times and adjuvant therapy as is the use of a derma roller but do certainly give them a try before going onto hair transplant.
