Hi Your son is 14 years, weight is 51kg.... Even if you consider paracetamol of 15mg/kg that also comes around 750gms. Instead you can give 500mg tablet or 650mg tablet orally instead of syrup.. It is always good to treat root cause of fever rather than choosing paracetamol or mefenamic acid
Next Steps
feel free to consult me on practo for further evaluation and management

At 51 kg your son should be on a proper paracetamol dose, not syrup - 500 to 650 mg tablet every 6 hours when fever is above 100, maximum four doses a day. Dolo 250 syrup 10 ml gives only 250 mg, which is why the fever keeps coming back; that is an underdose for his weight. I would avoid Meftal-P (mefenamic acid) in a child with fever - it commonly upsets the stomach and is not needed when paracetamol is dosed correctly. Give sponging and plenty of fluids alongside.
Next Steps
If fever crosses 3 days, get CBC with dengue and malaria tests done. Feel free to consult me on Practo with the reports and I will guide the treatment.
