Thank you for sharing your concern. Having a consistently elevated blood pressure of 170 and occasional spikes to 195, even while on medication, is a cause for attention. Telma (Telmisartan) is a commonly prescribed medication for hypertension, but it may not be adequately controlling your BP, especially with these occasional spikes. To manage this, here are a few important suggestions: 1. **Medication Adjustment**: Sometimes, adjusting the dosage or adding another antihypertensive medication may be necessary. It's important to consult with a doctor for any medication changes.    2. **Lifestyle Modifications**:    - **Diet**: Incorporate a low-sodium diet. Reducing salt intake can significantly impact blood pressure. Focus on a diet rich in fruits, vegetables, and whole grains.    - **Hydration**: Ensure adequate fluid intake, but avoid excessive intake of caffeinated or sugary beverages.    - **Exercise**: Regular physical activity, such as brisk walking or light aerobic exercise, can help reduce BP. Aim for at least 30 minutes most days of the week.    - **Stress Management**: Practice yoga, meditation, or relaxation techniques to help control stress, which can contribute to spikes in blood pressure. 3. **Regular Monitoring**: Keep a daily record of your blood pressure readings at different times to monitor fluctuations and share this data with your healthcare provider. 4. **Avoid**:    - Excessive consumption of fatty or spicy foods.    - Smoking and alcohol, as these can exacerbate hypertension. For better evaluation and personalized treatment, I strongly recommend consulting with a physician in person, as a complete medical history and physical examination are essential. Please don't take haphazard treatment via tele-consultation. It should be a proper consultation with a General Physician in his clinic. Hypertension is a silent killer. It has no symptoms, most of the time it is diagnosed during a routine health checkup or during the treatment for some medical emergency. Telmasartan is a single dose medicine. You may be in need of more than one medicine. And inadequately treated Hypertension may lead to severe complications. Please consult a General Physician or a Cardiologist in your area for the proper control of your hypertension.

Telmisartan 80 mg has not been able to control your blood pressure.Next step is to add a medicine from different antihypertensive like amlodepine/diuretic to bring the BP to an optimal level .
