She has a decay, it needs to be treated, also she has high risk for caries in other tooth . Please get her a remineralisation paste ( Kidodent or Cheerio red) and  application of flouride varnish by pedodontist.

Treatment with decayed tooth should be done. Treatment to be decided after examination and x ray. It is very common that children don't brush teeth at this age. Brushing twice daily is necessary. Kindly go through the blog mentioned in the link below.

Hi. She has a decay on the molar. Its necessary to restore that tooth immediately before it leads to any further damage. Any infection if not treated may reach the permanent tooth bud and may lead to abnormalities in that so please get it resolved soon.  Also electric tooth brushes are available in the market which are easy to use and many children are fascinated by them. It is common for children to not brush their teeth. You could find flavoured tooth pastes like strawberry which motivates the child.

As she is just 4 u need to take her to a pedodontist and get  all her teeth evaluated .decay is present u need to get it filled as we need to save the teeth till the time it exfoliates naturally . So get it restored . It's difficult to convince  kids for brushing but u need to get it done by using attractive brushes and tooth pastes and showing them the videos or animations that  shows importance of brushing .
