Hlo sir…after evaluating your x-ray,i come to the conclusion that the respective tooth has caries on the side from where 3rd molar have been extracted. As it is most probably caused by food entrapment between your 3rd molar and the respective tooth.As u r experiencing pain while having cold food that is mostly because of exposed pulp and that u can get evaluated by your dentist after further examination. Mostly the tooth will go for Root Canal Treatment.

Consult your dentist and get the treatment done as the pain will worsen with time only if u will delay the treatment.

Hii sir if you are having pain then better to go for root canal treatment. You can also discuss with your dentist regarding the treatment plan. But delaying the treatment cause worse consequences.

The pain you describe — only while chewing tightly and with cold food for 2–3 seconds — can happen when the tooth has decay, especially on the back/distal side. The short-lasting cold pain usually suggests that the nerve is irritated but may still be healthy. However, pain on biting should also be checked for a crack or pressure on the tooth.
Next Steps
* Please get the tooth examined by a dentist.
* If the decay has not reached the nerve, removing the decay and placing a filling may solve the problem.
* If the decay is very deep and the nerve is affected, root canal treatment may be required.
* The previously removed wisdom tooth is unlikely to be the main issue now; the decay in the adjacent tooth needs attention.

* Avoid very cold foods/drinks temporarily.

Hello sir
After evaluation of your xray. I can see a deep cavity in the adjacent which is present adjacent to your extraction space. This is very common finding as tilted wisdom cause food lodgement which results in cavity in adjacent teeth. Most probably this will go for root canal as exposed nerve is causing pain and sensitivity. That has to be excavated and evaluated at the time of treatment.

Hello!
The pain Seems to be from the distal aspect of the second last molar.
Would advice to avoid extreme hot or cold food or beverage from that side.
Also, get it evaluated by the dentist. Requires deep filling/ root canal treatment.
