Deep cut on foot
U have a deep cut on the gap of two finger of foot while swimming.The same occurred due to engage of my nail.I have used hydro heal ointment for first two weeks and antibiotic for 5 days. It has been a month but the wound is not healing itself . I am very much worried what to do kindly tell me I have attached the starting day and today's picture here

Looks like the skin has become macerated likely from all the creams etc you have been applying. Keep the area dry and only use dry gauze to cover when outside the house. Rest of the time, leave it to dry. It will heal faster. All the best. J G S R Clinic

The cut between your toes is not healing properly because of three reasons that are very common in this area: 1. Moisture trap between toes – this keeps the wound from drying and delays the formation of new skin. 2. Constant movement and friction while walking – the wound keeps opening again from the sides. 3. Possible mild fungal or mixed infection – very common between toes, especially after swimming. Even though you used antibiotic ointment and an oral antibiotic, the wound has not closed because medicines alone cannot heal a wound that remains moist and is repeatedly stretched during walking. The good news is that your wound does not look dangerous, but it needs proper wound-care steps to close completely.

1. Keep the area completely dry After bath, dry between the toes using tissue or cold hair dryer. Moisture is delaying healing. 2. Use a combination treatment for 7 to 10 days Twice a day: • Apply Mupirocin ointment on the deeper part and • Apply Clotrimazole cream on the surrounding skin This treats both bacterial and fungal components. Do not cover with a very tight bandage. Use a small sterile gauze lightly placed between the toes to keep them separated. 3. Reduce friction Place a small cotton roll or gauze between the two toes while walking. This stops the wound edges from re-opening. 4. Avoid swimming or soaking the foot No water exposure until it heals. Swimming pools easily cause delayed healing and fungal growth. 5. Use a wound-healing dressing If possible, use silver nitrate hydrocolloid dressing or soframycin-soaked gauze changed every 24 hours.

A wound between toes that is not healing after 1 month usually means infection, moisture, or friction is preventing healing. Do this now: Keep the area dry – After bathing, wipe well and place a small cotton gauze between toes to reduce moisture. Use an antibiotic cream – Mupirocin ointment or Fusidic acid cream 2–3 times/day for 7–10 days. Avoid hydro-heal now – Moist dressings delay healing between toes. Clean daily – Wash gently with mild soap + warm water → dry fully → apply ointment → keep gauze. Check for infection signs – Redness, pus, bad smell, increased pain = needs doctor visit. If diabetic or poor blood circulation – MUST see doctor, as toe wounds heal very slowly.

Blood test for fbs, ppbs2 , crp, cbc, esr. Extra tissue  must be treated by a well educated  compounder. Use T bact ointment, its an antibiotics  use after dressing by compounder. Tab zerodol sp three times a day after food for five days.  Montek lc one at night for five days.

The wound is not healed yet being one month old which it should have been otherwise in a normal case. You need to monitor your blood sugar levels. Kindly do a fasting and pp sugar test. Your age may not indicate diabetes but you need to rule out this first. And then, you will need to take proper foot care. Please feel free to consult.
