Thanks for sending across your query. After proper consultation and taking a complete medical history, we will first advise necessary investigations. Once all reports are reviewed, we will decide your medication if any abnormalities are found Along with this, you will be advised lifestyle modifications including walking, gym exercise, and a structured diet plan. If your weight is above 120-125 kg and you are unable to lose weight despite regular exercise and diet control, and you continue to experience symptoms like breathlessness or obesity-related issues, then surgical options may be considered. These include procedures such as Mini Gastric Bypass (MGB), Laparoscopic Sleeve Gastrectomy (LSG), and Roux-en-Y Gastric Bypass. After these procedures, significant weight loss can be achieved.

Feeling breathlessness and fatigue during physical activity can be related to excess weight, but it is essential to rule out underlying causes such as anemia, vitamin deficiencies (like Vitamin D or B12), or cardiac/respiratory issues. Obesity increases the workload on your heart and lungs, which leads to the symptoms you are describing. Improving fitness is a gradual process that involves both metabolic assessment and lifestyle modification. Next Steps Baseline Investigations: I recommend getting a CBC (Complete Blood Count), Lipid Profile, HbA1c (Diabetes screen), and a Thyroid Profile (TSH) to check for metabolic causes of fatigue. Physical Assessment: Given the breathlessness, an ECG and a clinical chest examination by a physician are recommended before starting a vigorous exercise program. Gradual Activity: Start with low-impact activity, such as a 15–20 minute brisk walk daily, and gradually increase the duration as your stamina improves. Dietary Review: Focus on a high-protein, high-fiber diet while reducing refined sugars and processed carbohydrates. Helpful Tips / Word of Caution Listen to your body: If you experience chest pain, extreme dizziness, or fainting while moving, stop immediately and seek medical attention. Consistency over Intensity: For sustainable weight loss, consistency in your daily movement is more important than occasional intense workouts. Hydration: Ensure you are drinking adequate water (2.5–3 liters a day) as dehydration can often mimic feelings of tiredness and exhaustion.
