I have been facing floating sensation for past 2 years reduction in libido, pins and needles pain in palms and feet and some time severe pins and needle pain in penis after ejaculation Already visited many doctors almost all of them are telling that my blood report is normal but my symptoms are not getting improved come and goes not able to judge what is happening. Homocysteine >50 — this is very high (normal is under ~15). Homocysteine rises when either B12 or folate is insufficient, because both vitamins are needed to clear it. A level this high strongly suggests significant deficiency of one or both. Folate 1.19 — this is low (normal usually >3-4 ng/mL), confirming folate deficiency is also part of the picture. Ferritin 32.3 — low-normal, borderline low iron stores, which can independently contribute to fatigue/lightheadedness.

Yes, functional B12 deficiency confirmed — high MMA/homocysteine + low active B12 despite "normal" serum B12 explain your nerve symptoms. Needs proper IM B12 loading (not oral), plus nerve conduction study if symptoms persist. Book a private consult to plan the injection protocol.
