Anal fissure
My sister is been having anal fissures since one month. She's been taking medication since 15 days. Medication includes - anorelief ointment twice a day, cremaffin syrup, enzoheal when the pain is severe. After medication her problem reduced by 70% almost. But now the condition is becoming like before slowly. She noticed pain increased a bit. After so many days of treatment also she's not able to get rid of this pain. It's affecting her a lot. She gets scared to pass stools. I have read that botulinum toxin injections are also an option other than surgery. So are botulinum toxin injections still given? Or is surgery the only way to go? What is the recurrence rate after surgery?

Anal fissure is a kind of injury and constipation hard stool is one big factor in it. So there are two parts of treatment. One is healing and second is prevention of further injury. Apart from medicines she is using botulinum toxin or botox is one option. It can heal but effect last about three months. If it recurs surgery is final way to treat.
But there is conservative way to treat and get rid of it completely without surgery. Do following
- avoid constipation and hard stool
- do not delay stool
- do not strain at stool
- take sufficient fibers and water
- use stool softeners if required
- avoid too spicy food
- if intestinal infection or diarrhea occurs get it treated quickly
