Low baseline BP of 100/60 that drops further with exercise, along with inability to gain weight despite eating and persistent fatigue, suggests your body may have an underlying issue affecting both metabolism and cardiovascular regulation. Possible causes include adrenal insufficiency, thyroid dysfunction, nutritional deficiencies (iron, B12, Vitamin D), or autonomic nervous system imbalance. These need to be ruled out systematically.
Next Steps
Get a blood test including thyroid (TSH), cortisol (morning), CBC, ferritin, Vitamin D, B12, and blood sugar done. Consult a physician in person for a proper evaluation. Please consult me directly on Practo for a detailed evaluation.
