Due to itching it got wound , unable to walk properly , mild pain , previously 2 months back it got cured and again same issue please help

From the image, this looks more like inflamed eczema / dermatitis of the foot with fissuring, though fungal infection can also present similarly and needs evaluation. Avoid scratching and avoid self-use of mixed steroid creams. Keep the area dry, moisturise regularly, and avoid friction from tight footwear. As this has recurred after healing and is affecting walking, I would recommend a dermatology consultation for proper diagnosis and targeted treatment.
