Based on standard antiparasitic protocols for enterobiasis (pinworm infestation), antihelminthic medications kill adult worms but do not eradicate microscopic eggs that have already been laid around the perianal area. Because newly laid eggs take approximately 14 days to hatch into new adult worms, a second repeat dose is universally scheduled exactly 2 weeks (14 days) after the first dose to break the parasite lifecycle and prevent auto-reinfection. However, for an accurate clinical diagnosis and to rule out other causes of anal pruritus, malabsorption, or persistent bloating, a formal medical evaluation is necessary.
While intestinal parasites can cause localized pruritus and mild abdominal discomfort, chronic inability to gain weight accompanied by chronic gas and bloating often warrants investigating other underlying gastrointestinal factors, such as irritable bowel syndrome (IBS), small intestinal bacterial overgrowth (SIBO), celiac disease, or baseline metabolic conditions.

TAB Albedazole 400 mg 1st day
Look for warm in stool if warm find in stool then have to repeat 3 dose at intervals of 2 day ..
If warm is not seen then complete the dose 1 tab per week for 3 week .
No need to repeat Ivermectin...
For pinworms the single dose does not finish the job because eggs from the bedding and nails reinfect you. Repeat albendazole 400 mg as a second dose 14 days after the first one; ivermectin is not needed again. The whole household should take the same single dose on the same day, since silent carriers are the usual reason itching keeps returning. Trim nails, hot-wash bedding and inner wear, and wash hands after using the toilet for the next 2 weeks. Bloating and no weight gain despite good diet is not only worms, so thyroid, coeliac screen and haemoglobin deserve a check.
Next Steps
Second albendazole 400 mg dose exactly 2 weeks after the first, same dose for everyone at home, plus strict nail and linen hygiene. Get CBC, TSH, tissue transglutaminase IgA and a stool test. If itching or bloating persists after the repeat dose, book an online consult with me and we will look for the real cause behind the poor weight gain.
You need proper consultation . At this age the above mentioned symptoms may not be because of worms .
You need detailed history , usg and few blood tests .
If the first dose of albendazole was taken this Monday, the second dose for pinworm is usually after 14 days (same day next week after next). Ivermectin is not always required for pinworm.
Also do these, otherwise it comes back:
- wash hands with soap, especially after toilet and before food
- keep nails short
- morning bath and change underwear daily
- wash bedsheet and clothes in hot water
- treat household contacts if the doctor advises
Not gaining weight and bloating are not always due to worms. If these continue after the second dose, get stool test and a proper check-up. Do not keep repeating deworming every few days.
