I am Dr. Shamoon. What you are describing - profound weakness, bone and body pains that flare when you lie down in bed, headache, but no fever - is the classic post-dengue convalescent phase, and at one week out it is expected rather than alarming. Dengue transiently suppresses the bone marrow and causes a lot of muscle protein breakdown and fluid loss, so the fatigue and "break-bone" aches typically linger for 2 to 6 weeks even after the virus has cleared. The fact that your fever has settled is the most reassuring part of your message. Two things commonly prolong it in someone like you, a 28-year-old IT professional: sitting straight back at a desk too soon, and poor intake of fluids and protein during the illness. What I would want to make sure of is that this is only convalescence and not a lingering problem - persistent low platelets, anaemia from a small bleed, dehydration, low sodium, or a post-viral effect on the liver or thyroid can all present exactly as unrelenting weakness. Post-dengue headache with body pain can also simply be dehydration. A simple repeat blood test settles this in a day. Do not take ibuprofen, diclofenac, aspirin or any NSAID for these body pains - only paracetamol is safe until your platelets have fully recovered.
Next Steps
1) Get a repeat CBC with platelet count, plus haematocrit, serum electrolytes (sodium/potassium), creatinine, LFT and TSH this week - one week after dengue this is standard and it will tell us whether your weakness is simple convalescence or something correctable. 2) Fluids are the single biggest fix: 3 litres a day of water, ORS, coconut water, lime water, buttermilk, soups and fresh juices. Keep urine pale. 3) Protein repair: eggs, dal, paneer, curd, chicken or fish at every meal, plus small frequent meals rather than three big ones. Add fruits rich in vitamin C and iron-rich foods. 4) Rest properly for another 1-2 weeks - stay off gym, running, heavy lifting and long screen shifts. Return to work gradually, and take a 5-minute break every hour. 5) For headache and body pain take only paracetamol 500-650 mg, up to three times a day with food. 6) Change position slowly when getting up from bed, as post-dengue blood pressure drops are common and cause dizziness. 7) Expect steady improvement week by week - if you are not clearly better in 2 weeks, or the headache becomes the dominant symptom, get reviewed in person. You are welcome to share your reports with me on a Practo consult, or see me at Synergy Multispeciality Hospital, Miraj, for examination and a recovery plan.
Health Tips
Go to hospital immediately if you notice bleeding from gums or nose, red spots on the skin, blood in vomit, stool or urine, black stools, severe abdominal pain, persistent vomiting, breathlessness, cold clammy hands, fainting, very low urine output, or if fever returns. Strictly avoid ibuprofen, diclofenac, aspirin and alcohol until your platelets are confirmed normal.
