Underweight
My daughter is underweight. She is 2 years 11 months old, but her weight is still 10.3kg. Her height is 87cm. She reached this weight when she was 1.5 year. Since then there is no increase in her weight. She eats everything not a picky eater, she is very active and plays well. I consulted 2 doctors both said she is fine. But I want to know what is the reason she is not gaining weight. Please suggest.

1. I understand your concern and I am medically concerned as well, because lack of weight gain for a long period always needs proper evaluation and follow-up, not just reassurance. 2. At 2 years 11 months, your child’s weight and height are proportionately low, which usually points toward a constitutional growth pattern, but medical causes are always considered and ruled out carefully. 3. The absence of red-flag symptoms such as poor appetite, repeated infections, chronic diarrhea, vomiting, lethargy, or developmental delay makes conditions like malabsorption, chronic infection, or hormonal problems less likely at present. 4. Since your child is active, playful, eating well, and developmentally normal, the most likely reason is high activity level with low calorie density intake, rather than an underlying disease. 5. Treatment at this stage is nutritional, not medicines: increase calorie density by adding ghee/butter to meals, full-fat milk and curd, paneer, peanut butter, egg, and calorie-rich smoothies, without force-feeding. 6. I would advise regular growth monitoring every 3 months, and if weight gain does not occur or any warning signs appear, we will investigate further and treat accordingly at the right time.

Your child’s height is appropriate for age. The weight is on the lower side, but it is still within an acceptable range. The most common reasons for relatively low weight gain at this age include: 1. High activity level 2. A normal slowing or plateau of weight gain after 1.5 years 3. Mild micronutrient deficiencies, especially iron What you can do: • Offer an energy-dense diet, including foods like paneer, ghee, eggs, and dry fruits • Ensure a balanced diet with adequate proteins and calories • Monitor growth every 3 months When to worry or seek re-evaluation: • Actual weight loss • Chronic or persistent diarrhea • Recurrent infections • Any delay in developmental milestones At present, close monitoring and nutritional optimization should be sufficient.
