Samonela typhoid O and H are coming reactive , upto titre 1.40 and 1.16 respectively in the lal lab test report should we start medicines , esr is also 48

A Widal at 1:40 and 1:160 is a weak result and in an endemic area it can stay positive from an old infection or even a vaccine. I do not start typhoid treatment on Widal alone. A raised ESR only says there is inflammation somewhere, it does not confirm typhoid. What decides it is whether she is actually running fever, and blood culture is the test that gives a real answer.
Next Steps
If she still has fever, get a blood culture plus CBC and CRP before any antibiotic is started. No fever, no symptoms means no treatment needed for this titre. Fever crossing a week, drowsiness, vomiting or abdominal pain means she should be seen the same day. Message me on Practo with the reports and I will tell you clearly whether medicines are needed.

Kindly visit a hospital and get blood cultures and a cbc. You most probably wont need in patient admission. But based on your test results, the necessary medications can be prescribed to you

Please share your symptoms, because it is disease which is treated, based on symptoms,  detailed history and Test results. The report is suggestive of typhoid, but treatment in detail will be based on detailed history.

If 1:160 then positive. Advice :- 1) Bland diet 2)Avoid tea coffee alcohol oily spicy junk food 3)If fever present:- Tab pcm 650mg twice daily after food. 4) Tab cefixime200mg - twice daily for 7 days. 5) Tab bifilac - twice daily for 5 days. 6) Drink plenty of water mostly ORS - 3 litres daily

Sometimes widal test is non-specific. So treatment should be depends on symptoms.if you feel symptoms like fever,diarrhea then starts antibiotics.

Sirf Widal report ke basis par typhoid confirm nahi hota. O 1:40 aur H 1:160 titres ko symptoms ke bina typhoid nahi maana ja sakta. ESR 48 bhi nonspecific hai aur apne aap antibiotic start karne ka reason nahi hai.
Next Steps
Agar patient ko persistent fever, headache, abdominal discomfort, loose motions/constipation ya weakness hai, to physician examination karwayein. Possible ho to blood culture with sensitivity, preferably antibiotic start karne se pehle, karwana better rahega.

Filhaal bina proper clinical evaluation ke anti-typhoid antibiotics start na karein. Adequate fluids aur light diet rakhein. Persistent/high fever, severe abdominal pain, vomiting, confusion ya marked weakness ho to promptly doctor ko dikhayein.
