For bilateral Grade 3 varicocele with persistent pain, microsurgical varicocelectomy (subinguinal approach) is the gold standard — it gives the best long-term pain relief (~70–80%) and lowest recurrence. Embolization is a good minimally invasive alternative but has slightly higher recurrence rates. Homeopathy/Ayurveda have no proven efficacy for structural Grade 3 varicocele.
