My main concern is fertility preservation and whether conservative management can still work in my case.Ultrasound / Scrotal Doppler:
* Grade III varicocele on both sides
* Right testis: 3.9 × 1.7 × 2.4 cm (8.1 cc)
* Left testis: 3.8 × 1.7 × 2.4 cm (8.6 cc)
* Vein diameter:
* Right: 3.2 mm
* Left: 3.8 mm
* Reflux present during Valsalva
* Dilated veins seen mainly in standing position
Symptoms:
* Mild right-side pain started around end of March
* In the last 2 weeks pain became bilateral and more noticeable
* Pain is tolerable and does not disturb sleep
Semen Analysis:
* Volume: 3 ml
* Sperm concentration: 72 million/ml
* Total sperm count: 216 million
* Total motility: 83%
* Progressive motility: 79%
* Morphology (normal forms): 2%
* Diagnosis: Teratozoospermia
Hormones:

FSH: 5.28 (normal)

LH: 5.67 (normal)
*
Testosterone: 3.46 ng/ml (normal)
How many cases have you seen where surgery/embolization was NOT done and fertility remained good long-term for Grade 3 Vericocele
