Hi mam..    The hole in the teeth is cavitation due to caries...   First and foremost x_ray(IOPA) of the cavitated tooth should be taken to fule out the extent of the lesion... If the cavity is superficial you need to go for simple filling If the cavity is deep pulpectomy of the primary teeth should be done
