Previous filling needs to be replaced by doing RCT , 2 wisdom teeth needs to be extracted if pain.

White thing on your tooth is an old filling, there is secondary caries below the filling which might have involved the pulp there bycausing pain , xray not clear but looks like upper second premolar also having caries along with the opposite side lower third molar , consultation  might give a better picture if they need filling or rct
