Submucosal fibroid
4 fibroids alongwith one submucosal fibroid of 18 mm and another outer one of 79 mm. They are increased in size from last year. Why Endometrial thickness is 15 cm on 19 day (in a 25 day cycle). Is it necessary to undergo surgery for fibroids at 40 years of age? symptoms are there of heavy bleeding and stomach ache.

An 8 cm fibroid is quite a considerable size. Also since you are symptomatic it is a good reason for surgery. However if you want if symptoms are not too severe, you can try medical management for a few months and see.

As you are symptomatic, we need to undergo some treatment. For Raised endometrial thickness on Day 19, you can get a repeat USG done after menses and check for Endometrial thickness. If it's in normal range, you can opt for hysterectomy (removal of uterus) surgery directly. But if it's raised even after menses, then endometrial sampling should be done before going for hysterectomy. D&c procedures along with removal of submucous fibroid can be done hysteroscopically. And if it resolves all your symptoms, then hysterectomy can be avoided

Hi, If you are symptomatic and if the symptoms are deteriorating your quality of life by loss of blood in turn causing anemia, It is a good idea to get rid of it by surgery. Medications will temporarily help and are not the solution. Surgery is definitive treatment esp for the submucosal fibroid

Since you have heavy bleeding and pain, it is better to get treated. Submucosal fibroids are usually difficult to treat with medicines alone and will need surgery if medical management failed. Since you also mentioned a thickened Endometrium, it is better to do an endometrial biopsy before proceeding with surgery.
