Speech delay
Hello Doctor Am worried about my Child...He is 3.11 years old and he will be 4 in a month He does not speak constructive sentences and his activities and responses are good he understands he ..till now not into a constructive speech he just expresses his feelings by showing hands he speaks two letter words and he repeats what we say ...he is hyper active only he enjoys to mingle with people...we are trying at home to make him speak sentences but they are not constructive from his side  he also identifies some names which we taught him when ever he sees them he says them I hear lot many doctors saying less screen time less screen time but that really matters? Is there any Autism symptoms  And without screen you know with present generation kids it's not working out... Please Suggest remedies Thank you

Hello I understand your concern for your child According to the history you have provided your son doesn’t seem to have features of autism, and other aspects of milestones are normal Yes it’s true that screen time adversely affects development of child. For less than 2year old no screen time, and 2-5 year old only 1 hour screen time per day is allowed, that too for video call/socialisation purpose only For language delay, first we need to rule out hearing issues, then find other causes Hoping that he didn’t have any issues during birth or later, he should be improving with speech therapy alone

You should get a detailed consult to rule out autism and adhd. If only speech is affected and baby plays well, communicates well, and does not show excessive shyness and social reservation, autism is less likely. Nonetheless a detailed evaluation is required. Baby will benefit immensely from speech therapy and behavioral therapy if started at the right time.
Next Steps
do not wait before getting a detailed evaluation as every delay will also delay cognitive development.

Since the Covid pandemic lot of social factors have pushed the parents to offer phones, iPads and TV screen to keep their children occupied and self learn. ADHD and poor language acquisition in toddlers is a growing concern. There is no replacement to human interaction and language learning through parental and social support. The IAP guidelines state no screen time till 2 years of age and < 1 hour between 2-5 years of age I will suggest you to go stepwise. Consult a speech and language therapist. Before that a simple audiometry test will rule out any hearing problems. I doubt it sounds like autism. But once the above is taken care of, appropriate guidance into a developmental clinic can be sought

If he mingles well, has co-playinh tendencies he is less like to be autism spectrum but we need to rule out other behavioral issues like ADHD if so
