Hot Fomentation. Gentle Shoulder Stretching. Core Strengthening Exercises. Gentle Neck Exercises. Avoid sleeping on affected side.. Warm up and cool down is a must.

Hello! I realize how important is for you to exercise. At 250 sugar level you can not ignore. Joining gym is a good choice but you should follow your pace. Don't compare with anyone and be gradual.
Next Steps
The shoulder pain could be a cause of local injury to the joint. Get it healed with physio sessions. May be for shoulder you might need a different protocol for exercise and pain management
