Thank you for your detailed history. However to sort your particular issue out  will not be easy and more details on various aspects of your medical, family, personal history will be needed together with some in-person clinical examination or at least some good clear pics if one cannot come to the clinic. Urticaria is a broad diagnosis; Aquagenic Pruritus may be another. Do consider an Online Consultation ASAP 🙏
Health Tips
Avoid Home remedies and perhaps stress busters like Yoga may help calm you down. Stressors are common aggravating factors. A more sedative and anti-anxiety antihistamine like Atarax at night may be more suitable than cetrizine but best to visit your Dermatologist as it's been 3 months + as you mentioned on cetrizine with temporary respite.

Hello. From what you have told it sounds a little bit like hives. It usually goes away with antihistamines like cetrizine but can recur. But will need complete history, previous medications allergies , and photographs for proper diagnosis and treatment.

Health Tips
Observe the temperature of the water after which you get the rash/itching. Safer to use lukewarm water for bath

Detailed history and clear images are required for correct diagnosis and treatment. Cetrizine is for symptomatic relief but doesn't cure the cause.
