There are many reasons for shoulder pain, but in diabetics it's very likely you have a stiff shoulder called Frozen shoulder.
There are lots of treatment options, in my patients i give them a pain injection and their pain goes away then I ask them to do physio and stretching exercises aggressively and open up the shoulder again.
But you have to get sugar under control otherwise nothing works.

Check whether you also have some restricted motion of shoulder joint ..
Mostly can be frozen shoulder..
Get some anti-inflammatory medications and physiotherapy
It might be adhesive capsulitis and better to consult an orthopedic surgeon for evaluation.
Blood sugar levels should be under control for early and better results.
