Severe hip pain / lower back pain
Hi . I am having severe hip pain /lower back pain/pain in buttock/ankle pain in left side of the body. I consulted many ortho doctors and did several physiotherapy sessions but not seeing any improvement. Hamstrings and calf muscles are very tight and not getting released with physiotherapy and the pain is increasing day by day. Recently started seeing issue with my ankle , because of that not able to stand and walk for more than 10 mins. Feels like left leg is not able to bear the body weight . Also hip internal rotation is restricted and not able to sit on the hard surface  . Can anyone suggest what could the issue ? Thanks in advance

Dear friend, I think there are multiple issues which we are dealing here 1. Issue with your back. Fact that pain hip back buttock and radiation to left side suggest it is a spine pathology. Also you are not able to bear weight on the leg suggest you might have weakness due to nerve compression. Hamstring tightness is a sign of nerve compression in the spine. Also, you cannot walk for more than 10 min suggest claudication which might be due to nerve compression 2. Ankle issue might be separate and may be due to some ligament issue. 3. Hip issue may be separate or may be related to your back. Even if your MRI and xray are fine doesn't suggest you are fine. As we are treating a patients symptom not MRI and xray. So it is very important to talk and see each other or check your symptoms to confirm my hypothesis. Better diagnosis can only give better treatment. There can never be a same treatment for every patient.

This type of pain  may be associated with spine or hip pathology mostly. As per the x ray and MRI images, both hip and spine appear to be normal.. Sometimes there is a possibility of early degenerative changes in hip joint like in AVN..
Next Steps
We can rule out the possibility of AVN femur head by getting one MRI done of both hip joints. You can also start with hot fomentation,  analgesic tablet, calcium and vitamin d supplements,  analgesic ointment for local application..

Avoid sitting down, avoid heavy work, avoid Indian toilet,  avoid forward bending..
