Back pain can be due to multiple reasons. MRI may not detect some causes of back pain. To identify pain generator in back, often a detailed clinical evaluation is sufficient. In cases like yours often the cause is myofascial pain like trigger points.
Next Steps
Consult a pain specialist for detailed evaluation and treatment options like IMS.
