These could be reason of your childhood brought up.. People around you would have not encouraged or motivated you for your actions and behaviour in your childhood which in turn has developed into your habit. so please visit a counsellor or a psychologist and understand yourself to resolve this..

For eye contact,begin with talk in mirror to yourself while looking in to your own eyes and prepare a clear introduction note and introduced yourself by looking in your reflection eyes directly, gradually keep adding more scripts. The other symptoms may indicate low self-image, start learning something new and creative. Just go in to public places like Park, gym. Etc
