Root canal or extraction?
I have severe pain in the gum line margin around the tooth in 47. Severe pain in that gum in distal side of tooth. I also have jaw pain on same side for 3 years now. No dental work done on 47 except several occlusion grinding adjustments has been done on it as Dr suspect my misaligned bite could be reason for jaw.Did several adjustments and ended up having uneven bite now.I didn't have any finding on 46 but it was painful for tapping,biting etc.So they did root canal &followed by several retreatments&nothing helped. Did ext 46,9 months ago and still have pain in that gum.Had several cbct&normal. Now I have pain in 47 gums.That tooth gum margin surrounding the tooth.Mostly on distal side.I had several grinding adjustments on 47 last year as that was hitting hard.Even now that tooth hitting hard and if I bite down on something on that tooth hurts sore.Tapping and rubbing doesn't hurt.I use night guard.I bite down with it on 47& it hurts sore. Do I ext or RCT ? hurts more when laying down

Hello sir/mam' As per the x-ray provided here [RVG], there is a retained root piece distal to 47. Still need to be confirmed clinically. Pain on lying down suggests pulpal pain. As per your history and series of multiple treatments you underwent, I will advise you to get it checked. Simple grinding of tooth to correct bite issues is not advisable as it may result in increased sensitivity and sometimes pain. Bite issues can be corrected after detailed clinical examination. T-scan is a digital modality available for pain associated with bite issues. Losing a tooth is just like removing a book from the shelf when everything is packed. It cascades a series of changes from discomfort to pain in long-term. So unknowingly, you start chewing from one side and completely ignoring the other side because of pain. There are 4 muscles of mastication [chewing] on both sides, which support and conduct chewing. Any discrepancy between the two sides in long-term results in bite issues.
Next Steps
First contact the dentist to get it thoroughly checked. When you get free from your current pain, plan replacement of missing tooth.
