I can understand your concern but I can't decide on using laser as I haven't seen the acne type.
Yes laser can help with post acne pigmentation and scars but it is not a permanent solution .I would recommend you to consult a good dermatologist to decide regarding laser as physical examination may be necessary.
Next Steps
Since u've already tried adapalene, azelaic acid, salicylic acid I would suggest using Glycolic acid face wash (Aha glow), adapalene+clindamycin ointment at night(avoid direct sunlight exposure). If you have very dry skin or combination skin use light weight, non pore clogging moisturizer (like Acnemoist cream).
Some tips:
Avoid greasy and oily food,do regular exercises.
Avoid exposure to dust and pollution.
Regular face wash.
Don't experiment with skincare .
Don't stress as it takes some time for skin to recover.
Health Tips
Do you have any other issues like irregular periods or excessive facial hair growth, hair loss or excessive weight gain?If so please do consult a gynecologist physically.
Since u've had the problem over long years it could be hormonal acne.
Need a picture. Consult a dermatologist (or me) with a picture or on video to exactly evaluate it properly and suggest you proper treatment that will work. You might not even need anything so don't take just anything by yourself.
Next Steps
Take a picture or come on a video call for proper evaluation and the right guidance.
Health Tips
Don't take just anything or get any procedure done without proper consultation and advice. So you need a consultation and a blood test too with testosterone levels added. Then show or consult me or any dermatologist.
Laser treatment do help with your active acne and post acne scars but the type of laser depends on the size and type of scars. For permanent and visible outcome you will have to go for variable number of sessions.
Initially you might see redness ,swelling post treatment and it is essential to use sun protection with spf50+ and other medical treatment following lasers for better permanent outcome.
If you are having acne for the last 5–7 years with active acne along with post-acne marks, please understand that laser is not a permanent cure for active acne.
Laser treatment mainly helps in: • acne marks
• pigmentation
• redness
• acne scars
• skin texture improvement
For active acne, laser alone will not give satisfactory results and in some cases acne may even flare up.
Commonly used laser options are: • Q-switched Nd:YAG / Pico laser – for acne marks and pigmentation
• Fractional CO2 laser or Microneedling RF – for acne scars
• Blue light / LED therapy – supportive treatment for active acne
Before starting laser, it is very important to control active acne with proper medical treatment.
There is no single permanent one-time solution, but with correct dermatological treatment, acne can be controlled long term and skin quality can improve significantly.
Next Steps
Consult a dermatologist in person for proper skin examination
• Control active acne first with medicines if required
• Decide laser type only after clinical evaluation
• Laser sessions are planned stepwise based on skin type and severity
Health Tips
Do not start laser when active acne is present
• Avoid self-medication and random cosmetic procedures
• Always get laser done from a qualified dermatologist
• Results need multiple sessions and maintenance treatment
Acne and acne scars are different issues:
1. Active acne (breakouts)
Caused by clogged pores, bacteria, inflammation, and oil production.
2. Post-acne marks/scars (PIH, atrophic scars)
Caused by inflammation damage and healing changes.
Creams help some patients, but if you’ve tried:
• Adapalene
• Azelaic acid
• Salicylic acid
and still see persistent acne or marks, laser can be effective — but no laser gives a permanent cure in a single session.
Lasers are tools that enhance skin structure and healing by stimulating collagen and reducing inflammation.
Next Steps
1. For Active Acne
Blue Light + Red Light Therapy (Photodynamic / LED)
• Reduces acne bacteria + inflammation
• Safe with minimal downtime
• Works best in mild-to-moderate acne
Fractional Laser + Phototherapy Combination
• Helps both breakouts and early marks
• Often used in clinical acne protocol
2. For Post-Acne Marks & Scars
Fractional CO₂ Laser (Ablative)
• Strongest option for deep scars
• Stimulates deep collagen remodeling
• Best for atrophic scars
Er:YAG Fractional Laser
• Less aggressive than CO₂
• Good for surface textural improvement
• Shorter downtime
Picosecond Laser
• Very effective for post-inflammatory pigmentation
• Improves dark marks more than deep scars
Q-Switched or Pico Laser
• Excellent for brown/red marks (PIH)
• Combined with other lasers for depth treatment
⸻
Does Laser Really Work?
✔ Yes, lasers can significantly improve acne scars and marks
✔ Fractional CO₂ is the most effective for deep scars
✔ Picosecond/Q-switched is best for pigmentation marks
✔ Combination approaches (laser + microneedling / PRP / chemical peels) often yield better results
But it’s important you know:
• No laser gives a “once and done” perfect result
• Usually multiple sessions are needed (often 3–6)
• Results vary by scar type and skin type
• Some downtime (reddening, peeling) is expected after stronger lasers
Health Tips
Before Treatment
• Avoid tanning/sun exposure
• Avoid harsh scrubs and retinoids 3–5 days before
• Consult a dermatologist experienced in lasers
• Always ask for before & after photos of similar skin types
During Treatment
• Pain can be managed with topical anesthetic
• Downtime depends on laser strength — plan accordingly
After Treatment
• Strict sun protection every day
• Use gentle moisturizers
• Expect mild flaking/peeling first week
• Keep follow-ups as recommended
Yes — laser can be very effective when topical creams alone are not enough.
Fractional CO₂ + Pico laser combinations are the most reliable for acne and post-acne marks.
But remember:
✔ Laser is not an instant permanent cure
✔ It’s part of a structured treatment plan
✔ Multiple sessions + sun protection + maintenance yield the best outcome
If you want, tell me:
• Skin type
• Scar type (pitted vs flat discoloration)
• Whether pimples are still active
I can give you a personalized laser plan with expected results and how many sessions you might need.
