One question is - is it perineal or perianal abscess? I am assuming u have been suffering from perianal abscess, because perineal and perianal are different. So you had perianal abscess, which u have undergone surgery, but you get pain and the surgery part swollens up now and then. It is most likely that you are suffering from fistula in ano. It's an abnormal connection between the skin and anywhere in the anorectum. Because of this connection, there is infection of the area and you get swollen. You have to take MRI Fistulogram to know exactly whether u have fistula or sinus and also the tract of the fistula fully.
Next Steps
MRI - Fistulogram. with Fistulogram report meet general surgeon for review and for further treatment.
