I had a surgery for perineal abcess back in April'17 post which the abcess reoccurred within 6months and then again after a year in Nov,19 now again after almost 6 months I am in pain at the very same surgery area it's swollen and paining but currently I am doubtful whether it's the abcess or anything else but I am curious to know why this problem is reoccurring and what is the solution for it ?

It's better u show to some good colorectal or genral surgeon
Do one MR FISTULOGRAM
IT WILL GIVE YOU THE DIAGNOSIS
If u do the conservative
Management it will again occur in 5- 6 month
So it's better do a well diagnosis and treatment as per

One question is - is it perineal or perianal abscess?
I am assuming u have been suffering from perianal abscess, because perineal and perianal are different.
So you had perianal abscess, which u have undergone surgery, but you get pain and the surgery part swollens up now and then.
It is most likely that you are suffering from fistula in ano. It's an abnormal connection between the skin and anywhere in the anorectum. Because of this connection, there is infection of the area and you get swollen.
You have to take MRI Fistulogram to know exactly whether u have fistula or sinus and also the tract of the fistula fully.
