I have strong pulsating toothache in my lower right molar when my tongue touches the tooth.
History - A part of the same tooth fell off last week while having a meal. This same tooth was partially decayed too due to cavity.

When part of tooth fell off last week , you would have visited dentist soon so that you would hav not got pain by this time , I think your innermost part of tooth called pulp has exposed and infected , hence you have a pain , thee tooth may go for filling or RCT , medication can only bring down pain but not a permanent solution , so necessary treatment has to be done , kindly consult near by Endodontist
