He may have Mixed Depression and anxiety, along with personality factor( social anxiety is associated factor) need SSRI BDZ Beta blocker and CBT etc...but over all on full evaluation Better to consult a Psychiatrist

Consult a Psychiatrist for a detailed assessment. It is likely with medication and counselings the symptoms will improve. If you are feeling suicidal or hopeless consult a Psychiatrist immediately.
