Residual empyema im the posteromedial aspect of the left the pleura with pleural thickening associated with lytic lesion in the left rib amd cavitating mass in the left lower lobe with left upper and lower lobe noduli Empyema with underlying malignancy ? Post TB changes/infective astiology.

Seems you may have carcinoma of the lung with either direct involvement of rib or metastasis. You need to consult pulmonologist and ask for Bronchoscopy with biopsy. Tuberculosis may present with similar symptoms so it will be next differential.
