This report don’t give us any clue as to why the Hemoglobin is low. That’s actually good to know that there is nothing serious. But still the patient needs evaluation for low Hemoglobin.

I don't know who diagnosed internal bleeding in the case of the patient and on what basis.  This report is of no value as far as internal bleeding is concerned. You should have consulted a Gynaecologist or a General Physician because your patient is a lady (at least this is what your sonography report says).

As per USG, the report is not serious...but the important concern is internal bleeding from somewhere as haemoglobin is 6 as per your statement...does the patient pass a black stool? Bloodstained vomiting?...the patient is also geriatric so there might be cancer-related causes...so needs proper evaluation and diagnosis under a good surgeon
Next Steps
kindly consult a surgeon
