The wisdom tooth on your left lower side seems to be decayed beneath the filling done. Root canal treatment can be advised for the same. Consult an endodontist to see if the root canal is possible since treating wisdom teeth requires proper access. If treatment not possible, get the tooth removed. You are having a secondary caries underlying your filling in lower left wisdom tooth and also interdental bone loss in between the lower left wisdom tooth and the adjacent tooth which may create problem in future so better you extract that wisdom tooth which will not give you further pain in future

IN the given OPG x ray film, the left lower third molar doesn't seem to be an impacted tooth. There is a deep filling in that tooth , pain may be due to that filling, or it can be due to impingement of the upper tooth to the tissues just beyond the lower third molar causing tissue inflammation and pressure on lower tooth.
Next Steps
consult with an endodontist first, if the left lower third molar cant be saved with root canal treatment ,then that tooth may need to be extracted.
Health Tips
if pain persist after extraction of that tooth , then upper wisdom tooth extraction may be also considered.

Hello No its not necessary. Please mention side of pain. Left or right. Rt side wisdom teeth required rct. Its look spread of cavity below restoration. Food lodged may possible on right side. But further review necessary as x-ray have there own limitation.you having fully erupted wisdom teeth. Problem may sove without removal.

mouthwash always help if its because of food lodged on that are. but if its because of cavity than antibiotic with restoretion or rct required. your dentist will check it and decide.

Hi...I have seen ur OPG..it shows old filling in left wisdom tooth and it is close to pulp..ur tooth position makes tough for RCT...so it is advised for wisdom tooth removal.
