From seeing the opg , u must get the wisdom tooth extraction as the upper wisdom tooth are impinging in the lower tooth area and lower wisdom tooth is also need to be extracted as it is completely horizontal and this may cause pressure on the adjacent tooth and cause caries sooner or later.
Looking at the OPG, the upper right wisdom tooth is impinching the mandible (the lower jaw) and the lower wisdom teeth are positioned horizontally which can cause this issue.
The pain can also run along the shoulder as you are experiencing.
Next Steps
Visit a dentist and get the wisdom teeth extracted

Hi,
Are you having any clicking sound while opening and closing your mouth or while chewing? The head pain could be related to the wisdom teeth.
It is less likely the neck, shoulder and hand pain is related to the teeth.
