A painful lump near the anus that has increased in size from a small, painless swelling could be a thrombosed external haemorrhoid, but it should not be assumed to be piles without examination. Other conditions, including an anal abscess or fissure, can present with a painful swelling.
Since you are also having difficulty passing stool, the immediate priority is to keep stools soft and avoid strainingâadequate fluids, fibre and warm sitz baths can help.
From an Ayurvedic perspective, treatment can be planned based on the type of Arsha, associated constipation (Vibandha) and the nature of the swelling/pain. However, because the swelling is enlarging and becoming painful, I would recommend an in-person anorectal examination before starting treatment. If you develop fever, pus, rapidly increasing/severe pain or significant bleeding, seek prompt medical evaluation.

Based on your description, this could be a thrombosed external hemorrhoid (piles), especially because it started as a small clot-like swelling, has increased in size, and has now become painful. However, other conditions such as a anal fissure, perianal abscess, inflamed skin tag, or less commonly another anal swelling can present in a similar way. An examination is important to confirm the diagnosis.
The associated constipation and hard stools can worsen hemorrhoids and increase pain by causing repeated straining during bowel movements.
Next Steps
Do not try to squeeze, puncture, or cut the swelling yourself. From an Ayurvedic perspective, treatment focuses on reducing constipation, improving bowel habits, and relieving local inflammation. from what you have described, it may be a painful external hemorrhoid, possibly with a clot (thrombosed pile), but it is not possible to confirm the diagnosis without examining you. Since the swelling has increased in size, become painful and is partially obstructing the anal opening, I would recommend an examination rather than assuming it is only piles.
Please avoid self-medicating or inserting any Ayurvedic/herbal preparation into the area before examination. Treatment depends on whether it is thrombosed hemorrhoid, fissure, abscess, or another condition.

Health Tips
In the meantime, keep the stools soft, drink adequate water, avoid straining and prolonged sitting on the toilet, and you may take a warm sitz bath for 10–15 minutes.
If you develop fever, rapidly increasing swelling/severe pain, pus, significant bleeding, dizziness, or difficulty passing stool/gas, please seek an in-person medical evaluation promptly.
This is most likely a thrombosed external haemorrhoid â a blood clot in an external pile. For Ayurvedic management: Triphala churna helps with constipation, and sitz baths with warm water are very effective for pain and swelling. Arshkalp Vati is a common ayurvedic haemorrhoid remedy. However, if the lump is very painful and blocking the opening, don't delay modern treatment.
Next Steps
Warm sitz baths twice daily, Triphala at night, high fibre diet, plenty of water. Apply Jatyadi oil locally for relief. If pain is severe or worsening, see a doctor for examination â drainage may be needed. Consult me on Practo for further advice.
Based on your symptoms, this is more suggestive of a thrombosed external hemorrhoid (blood clot) than simple piles, but a perianal abscess should also be ruled out by examination.
Meanwhile:
Take warm sitz baths 3–4 times daily.
Avoid constipation with a high-fiber diet, plenty of fluids, and a stool softener if needed.
Avoid straining during bowel movements.
Please consult a general surgeon/proctologist for examination, especially since the swelling is increasing and painful.
Seek urgent care if you develop fever, pus discharge, severe bleeding, or worsening pain.
Ayurveda treatment should be started only after confirming the diagnosis.
A painful, enlarging clot-like swelling near the anus should not be assumed to be piles. It could be a thrombosed external hemorrhoid, but an abscess or another condition also needs to be ruled out. Since it is increasing in size, painful and obstructing the opening, please get examined by a colorectal/general surgeon promptly. Avoid straining and self-medication; Ayurveda can be considered after confirming the diagnosis.
