Wax never causes pain until unless u try cleaning the ear urself. From history it appears as middle ear infection- Acute otitis media or has developed serous otitis media. U need to get the child examined by an ENT specialist and treated

As per your description it looks like he might have developed infection in the middle ear. Wax doesn't cause pain. Get an ent opinion for proper assessment and treatment.
