In vegetarians Vit B12 may be low due to poor intake (vit B12 is found in animal products only, including milk). Rarely absorption may be poor due to antibody or intestine abnormalities or alcohol use.
Dosage and route depends on the associated condition as mentioned above.
Raised Uric acid itself does not need medicines, diet modification can improve it. If joint pains is there only then you need medicines along with diet change.
Cholesterol and sugar can be managed by diet, exercise, medicines needed only if still abnormal after life style changes not helping.

Your Vitamin B12 is low and needs treatment. Uric acid is mildly high and can be managed initially with diet and hydration unless joint pain is present.
Rx:
Tab Methylcobalamin 1500 mcg once daily × 3 months
Cap B-complex once daily
Drink 3–4 litres water/day
Diet advice:
Avoid red meat, organ meat, alcohol; reduce sugar and oily foods.
Repeat B12 & uric acid after 3 months.
If joint pain/swelling occurs, medicines may be needed.
Consult a General Physician like me via Practo for follow-up and metabolic risk assessment.

For Uric Acid :-
While a uric acid above 8 is not ideal it does not always warrant treatment.
If you dont have any joint pain or history of renal stone or blood in urine(may point towards a current small renal stone) ,than prolong or lifelong treatement at these level might be unneccary.
For you a 6 week course of Tab zyloric (allopurinol) 100mg once day
Avoid red meat , alcohol , and grams (chana rajma etc.) for a 12 weeks.
Protein intake can be fulfullied via curd,pulses,1 whole egg or 2 egg white or paneer or chicken. These protein sources dont increase the uric acid levels.
Be on gaurd though a borderline cholestrol and prediabetes and raised uric acid are all signs and symptoms of poor metabolic health and need timely intervention.
At the age of 53 years these should not be ignored
