Gradually falling Platelet count can have numerous causes.First we have to rule put Platelet clumping - By looking on the manual Platelet count and if it's normal no need to worry ,if not needs further evaluation by looking at the peripheral smear. Platelets are mostly destroyed by spleen , so we have to check for Hypersplenism . Do u have another symptoms - petichae , any rashes, Fever? Alcohol intake ?
Next Steps
Take USG abdomen. ,Peripheral smear with manual Platelet count.

A gradually falling platelet count with high MPV (14) and no symptoms could suggest mild immune-related platelet destruction or vitamin deficiency. Since ESR is normal, nothing alarming now — but don’t ignore the trend. Next steps: Peripheral smear, B12/folate test, and monitoring. Consult if it drops further or symptoms start.
